A DC-9 has been de-iced twice, but in less than two minutes after take-off, both engines fail. As the pilots try restarting them the left engine erupts in flames, and then 3,200 feet above the ground, the plane quickly and silently falls from the sky. The plane breaks into three pieces, but remarkably everyone on board survives.
