  • Episode 1: Greek Ferry Disaster

  • Investigations expose a dark trail of negligence and guilt that resulted in the September 2000 sinking of the ferry the Samina Express off the treacherous rocks beneath the landmark rock formation, Gates of Paros.

  • Episode 2: Train Collision

  • A freight train charges onto an occupied single track, ploughing through the passenger train Super Continental. The traffic signals are found to be working perfectly, leaving only human fault to account for the tragedy.

  • Episode 3: Runaway Train

  • Before a freight train leaves the station, workers load the cars with some 2700 tonnes more weight. The conductor is never informed. Overloaded, he is unable to stop the train from derailing.

About the Show
The boat grinds against the jagged rocks, gouging open the side of the ship. As the ferry sinks in the midst of a raging storm, frantic passengers scramble to find life jackets and emergency rafts on the under-equipped boat. Many of the 472 desperate passengers are left thrashing in the two-metre waves, fighting to be rescued by local fishing boats that rush to the scene. As rescue efforts continue, the death toll climbs and questions multiply. And when experts begin to investigate why such an easily avoidable collision killed 80 passengers, they expose a dark trail of negligence and guilt. Many drastic human errors-including a captain who put the unattended ferry on autopilot and crew members who abandoned their posts to watch a soccer game-played disastrous roles in leaving the Samina Express lying beneath the sea.
